1. The DS-11 application form completed

Before completing the online application, please read the following crucial information: it is very important to all applicants. 

★ After accepting the privacy agreement on the first page, pick “Apply Online” in the bottom left-hand column of the following page, click “Submit,” and then continue with the application.

★ On the page labeled “Passport Products and Fees,” select “Expedited at Agency” under “Processing Methods” and “Standard” under “Delivery Methods” (Since Ambassador Passport and Visa will hand carry your documents to and from the U.S. Department of State, the mailing option does not apply to hand carry applicants).

★ Check the box next to “I have read and recognized the steps…” on the final page, under the heading “Next Steps” on the bottom left. Then, click the “Create Form” button, and then open and print your application. Sign only after you have appeared before the passport acceptance agent.

★ Application form DSP-64 must also be filled out and signed if your passport has been misplaced or stolen.

★ Both parents and the minor kid must be present when the paperwork is signed for minors under 16. The other parent must sign a consent form and have it notarized if just one parent attends:

2. Two current passport photos

Please note that the following passport images are acceptable: 

  • Photos must be 2 x 2 inches in size and taken no more than six months ago.
  •  The image should be between 1 and 1 3/8 inches wide from the bottom of the chin to the top of the head.
  • The images must be in color, and there must be no glasses.
  • Photographs should not be taken with a hat on or headgear that covers the hair or hairline, and they must be taken in full-face front view. Dark or nonprescription glasses with tinted lenses are not acceptable unless necessary for medical reasons. Homemade digital photographs are also unacceptable.

3. A completed and signed letter of authorization

Important information: For us to complete your application request, you MUST choose the first option (by checking the box on the left). Your passport application will take longer if you don’t do this. After choosing the first one, you must choose one of the following.

  •  When processing the passport application, the Passport Clerk shall place the printed, finished, and signed Authorization Letter (in blue ink only) inside a sealed envelope.

4. Verification of U.S. citizenship

One of the following is possible:

  • Certified Birth Certificate: This document is issued by the Bureau of Vital Statistics or the County or State Health Department. Birth certificates from hospitals are not recognized. Please click here for help obtaining a birth certificate.
  • Birth records from foreign consulates
  • Naturalization certificates also referred to as citizenship certificates
  • A previous, out-of-date U.S. passport was either issued when you were under 16 or more than 15 years ago.

Please note that the original birth certificate and prior passport must be supplied together when renewing a passport for a youngster under the age of 15.

  • Proof of Identity, as well as U.S. Citizenship documentation, is needed. A current driver’s license or state identification card issued more than six months ago must be on hand. Make copies of the front and back of the state identification card or driver’s License.

5. You must provide proof of your departure date from the USA

No proof of departure or travel is needed due to the pandemic until further notice. Nevertheless, if you have a travel itinerary, reservation, business letter, or another kind of travel documentation, kindly present it. A letter on the company’s letterhead certifying that immediate overseas travel is necessary, or a copy of the itinerary or airline tickets

7. Make copies of your documents before they are sealed.

Please create copies of the following before the documents are sealed (specified in the next step): 

  • All application forms, including the DS-11 and DSP-64, as necessary
  •  Copies of the front and back of the State ID or Drivers License (i.e., two sets of copies of front and back, one of which goes within the acceptance agent’s package); • Authorization Letter; • Proof of Travel (Travel Itinerary or Business Letter);

8. Completing the passport application

Bring the documentation mentioned above to your neighborhood passport acceptance agency (usually a post office, county clerk, or courthouse). Tell the acceptance agent if you are using a “hand carry” agent or a private agency to process your passport application. The clerk will seal the previous papers in an envelope and give it back to you (You must not open or tamper with this envelope). The sealed package must be mailed or delivered right away to the delivery office since it expires five business days after the day the documents are signed.

Please be aware that sometimes an acceptance agent will want to keep your sealed envelope. Please direct him to the “Hand Carry” portion of the Acceptance Officer Procedure Manual, or call Ambassador Passport and Visa Services if you run into this issue. Please be aware that the clerk will want the U.S. government to charge payment. Two checks should be used to cover this fee:

Please provide your birthdate and full name on each check. The sealed envelope created by the passport clerk should contain the following papers:

Please be aware that, unless expressly stated differently by an Ambassador Passport and Visa Agent, processing for a U.S. Passport starts on the FIRST BUSINESS DAY following receipt of all necessary paperwork.
